The forward facing cameras must be installed within 30 days of the effective date. The Insured can select its own cameras and comply with this Quote Condition by providing their QEO Loss Control Representative proof of purchase, provider name, or pictures of the installed cameras. The Insured can also select to obtain cameras at a substantially reduced rate through Motive. If the Insured selects Motive, they will sign a two year contract and will then receive a check out link from Motive to bill them $35.00 per month/ per camera. Please see attached documents for additional details within the QEO Fleet Guardian Program Flyer and to select a qualified camera implementation option on the Dash Cam Confirmation Form. If this condition is not complied with, the policy is subject to cancellation. o Continuation of coverage is contingent upon an effective risk control review of the insured's venture within 60 days from the time of binding.
