His role is to help improve employee productivity, business valuations, and EBITDA through innovative solutions and strategies. Wayne began his insurance career in 2003 as a producer in Georgia. Wayne graduated from Augusta State University, where he played college baseball. He grew up in Athens, GA and attended Clarke Central High School. Wayne and his wife, Kelli, have two beautiful girls, Avery and Kendall, and currently make their home in Athens. They are both huge UGA fans and members of Athens First United Methodist Church. This allows him to use his passion for serving others to insure they have a comprehensive risk management/insurance program that will protect the client, their business, and their employees. Brian is married to Angela and has three children. He lives on a lake so you will find Brian on his boat fishing; he also enjoys cooking and trying new recipes. Brian attended Auburn University of Montgomery while he served in the Air Force as a security police specialist. The forward facing cameras must be installed within 30 days of the effective date. The Insured can select its own cameras and comply with this Quote Condition by providing their QEO Loss Control Representative proof of purchase, provider name, or pictures of the installed cameras. The Insured can also select to obtain cameras at a substantially reduced rate through Motive. If the Insured selects Motive, they will sign a two year contract and will then receive a check out link from Motive to bill them $35.00 per month/ per camera. Please see attached documents for additional details within the QEO Fleet Guardian Program Flyer and to select a qualified camera implementation option on the Dash Cam Confirmation Form. If this condition is not complied with, the policy is subject to cancellation. o Continuation of coverage is contingent upon an effective risk control review of the insured's venture within 60 days from the time of binding. A QEO Risk Control Consultant will contact the insured and request a meeting after the policy is bound. Essential risk control 10/22/24 Page 6 of 25
recommendations or driver safety programs (if applicable) should be applied within 30 days of the review to maintain coverage. o Current MVR is required on all drivers including new and replacement drivers. Subject to acceptable MVRs on all listed drivers. The agent will be notified if any drivers are not acceptable for QEO's program o Driver requirement for all units: MAXIMUM AGE 72 unless pre-approved by underwriter o Driver requirement for light, medium and private passenger units: MINIMUM AGE 21 o Drivers must have a minimum of 2 years of experience driving equipment of a similar type, configuration and GVW o Signed and dated ACORD application o Signed and dated PIP selection/rejection form o Signed and dated Uninsured Motorist selection/rejection form o Subject to phone call with agent, insured, and underwriting within 5 days of binding to review QEO's claims reporting requirements o New drivers must be prior approved by underwriter o Addition of any equipment 15 years or older (model year 2010 and older) to policy requires prior approval of underwriter. Photographs required (front/back; both sides; interior), said photographs must demonstrate the equipment is in good condition o Subject to receipt of currently valued loss runs • Cyber o A completed TMHCC NetGuard New Business Application (NGPNBOA-62023), · signed and dated no more than 45 days prior to the proposed effective date. EQUIPMENT BREAKDOWN Provides physical damage coverage caused by sudden/accidental explosions, aka BOILER/MACHINERY arcing and other “breakdowns” of boiler, machinery, and electrical equipment. DRIVE OTHER CAR Provides Personal Auto Liability to named individuals who do not have a personal auto policy of their own (typically for owners, officers, and directors). DIRECTORS/OFFICERS Provides defense and settlement to protect officers and directors of an LIABILITY organization against damages from claims resulting from alleged negligent or wrongful acts. TRUSTEE/FIDUCIARY Liability protection for those who administer pension and welfare funds, profit- LIABLITY sharing, or other benefit programs against loss by errors or omissions. WORKERS COMPENSATION/ Provides the coverage mandated by the state or federal coverage for EMPLOYERS LIABILITY employers (based on number of employees) to provide for employees injured on the job. EMPLOYERS LIABILITY Provides Employer Liability coverage in those state-controlled work comp STOP-GAP COVERAGE programs that do not include employers’ liability coverage (OH, WY, ND, and WA). MONOPOLISTIC WORKERS If you have employees in these states, the employer is required to purchase COMPENSATION: Workers Compensation directly with the state and therefore coverage is not ND, WY, WA, OH provided under your workers compensation policy with us. UMBRELLA / EXCESS Provides additional liability coverage above those limits afforded by underlying LIABILITY coverage. Typically sits excess over general liability, auto liability, and employer’s liability coverage. Higher Limits are Available. POLLUTION COVERAGE Provides coverage for costs related to pollutants – sudden and accidental release of hazardous materials. Typical coverage includes cleanup and restoration as well as your liability for injury and death resulting from a Pollution occurrence. The policy can be issued to cover your premises, other’s premises, ‘over the road’, or for a specific job. PRODUCT RECALL Provides coverage for expenses associated with recalling a product from the market. COVERAGE DESCRIPTION CREDIT INSURANCE Credit Insurance should be considered when transacting with vendors overseas (ACCOUNTS RECEIVABLE) as well as here domestically. Credit Insurance reimburses bad debt losses from insolvency and bankruptcy situations. TRAVEL ACCIDENT Travel Accident Insurance provides accidental death and dismemberment benefits for employees while traveling abroad on company business, at the work site during their job, or on a 24-hour basis for employees.
